Depending on the leading style of NS, this just looks like a misplay. The ruffing the winner is automatic to get back to hand, but if the 8 of hearts looks like a doubleton, then S only has one spade (you know about 12 of his cards if N has only 2 hearts), so unless it's the K you're cold by leading the Q♠ on the first round. Declarer has played N for ♠Kx so the 8♥ would be led from 5 of them which is possible but I would have thought unlikely.
The other possible line is to play the lead as whichever of 98x/87x opps lead the 8 from and play the Q on the second round of spades playing S for 10x.
