If I change a setting - for example, turn on “confirm bids” — how do I save setting so I don’t have to re-set every time I sign on to BBO? BBO is lacking in this regard - why no Save button? Thanks.

It's saved automatically by your browser in its local storage. This (unlike cookies) should persistent indefinitely. So if it's not persisting, you likely have some custom browser add-on or security program which is overriding the normal browser behavior and clearing it.

Settings return to default on my browser (Chrome/mac) if I clear the cache - which is sometimes helpful.
It may also happen if you are using private browsing / VPN+TOR for example and preventing settings from being stored.
It's possible to avoid almost anything if you really try.

Settings persist on my Windows machine for each browser I use across reboots and reloads, regardless of which browser I'm using (I use Chrome, Avast, and Edge, depending). If I change something in Chrome, I do need to change it in Avast and Edge also... but once changed, it 'sticks'.
