I am not sure why my partner raised to 7C but she did. How should I play on a low heart lead?

I only see one real line - a red suit trump squeeze on East, hoping the hearts break 6-3 or worse and that East has the ♦K. It's a trump squeeze because South's only entry to the ♦Q in the end position is a heart ruff. For the setup we will draw trumps ending in North, cash ♥A pitching a diamond and ruff a heart, then play all but one trump pitching two diamonds. Now three rounds of spades ending in North and East is finished in the 3-card ending, if we can read the distribution:

East can't keep both ♦Kx and two good hearts, so they had to abandon one guard. If they blanked the ♦K, smack down the ace, ruff a heart, and cash ♦Q. If they went down to 1 heart, ruff a heart, diamond to the ace and the ♥9 is good. If I'm not mistaken the threat to ruff is essential for this type of squeeze, so we can't ruff a second round of hearts earlier in the play. Keep a close count of the hearts for this squeeze.

Ruff the lead and run five trumps pitching diamonds. Then cash three spades endind in dummy and the heart ace. Now you are in dummy with the 98 of hearts and the diamond ace, while on hand you have Q5 of diamonds and a trump.

If RHO has singleton diamond king at this point you cash diamond ace and hand is good; if he has two diamonds then he has only one heart so you ruff a heart and dummy is good.

This does require a bit of guessing as to RHOs shape, but you really only need to know the starting heart break and the lead will often help with that.
