In my version of the 4NT ask, a 5 reply shows A or no Ace (5 asks). Now 5NT is a specific King ask (to sign off in 5NT or higher opposite no Ace, go through the ask of 5 first), with 6NT as negative reply. If the Specific King reply is 6 or 6 then cue 6 asking for that King, which will elict either a positive 7 (so bid 7NT) or a negative 6NT (so pass).
It's not a frequent situation, but not much memory load either (if you already play cue over Specific King when trumps are established).

No - it's clearly a Very Strong hand. The spade *rebid* may be a psychic, but it's Natural - and anyway there are no restrictions on psyching rebids...
