Not an easy second call for East, who will fear only 18 total trumps. But having bid 1♥ immediately I wouldn't be in this mess (although quite possibly in some other one).

Same, but even more interesting if a 2 card club, as this type of hand (opening values and a chunky 4 card suit) is quite likely given that the weak hands are taken care of by 2 level bids.
We would bid (1♣)-1♥-2♠(fit) and now it would depend whether 3♣ is bid exactly how it would go but we'd discover no diamond control and play 4♥.

With the understanding that 2C is majors, I bid 4H. I don't think you can leave it to partner when you have such good hearts and an extra Ace, even KJxxx, Kxxxx, xx, x isn't totally hopeless.

It's borderline to me, even if OP did specify IMP.
But yes mikeh did have a point that we should know (and disclose) what partner might have.
