You hold:

AJT
AJ86
AJ94
42

and open a 12+-15 NT. Auction proceeds:

1NT P P X
P 2D P 2NT
P 3NT all pass

You lead a low heart (attitude) and dummy hits with:

xxxx
QT9
KTxx
J9

Dummy plays low, partner plays the 7 (std count) and declarer wins with the K. He now plays the 8 of diamonds to the K, partner playing the 6, and then a spade to the Q and your A, partner playing low. How do you continue?

This is strictly an inexpert opinion, but I would try A and another now. At the very least it keeps declarer playing the suits.
I'd rather have a bottle in front of me than a frontal lobotomy - but it might improve my bridge.

2 tricks in hearts + 1 spade + 1 diamond + 5 clubs = 3NT.

Partner has possibly a queen: I would return the 9
