After fourth-suit forcing

1 is explained as: Fourth suit forcing -- 4+D; 11+ total points.
Is it common for FSF not to be a game force?

2 is explained as: 4+C; 4+D; 11+ total points.
Why wouldn't GIB raise NT instead of misleading partner about his club support?

At another table, South's bid over 1 was 1N, explained as 3-5C; 2-5D; 2-4H; 2-4S; 12-14 HCP.
Doesn't 1NT deny a 4-card major? It must deny 5 diamonds.

Over that 1NT rebid, North bid 2 to show a heart stopper. Again, why wouldn't he simply raise NT, with both major aces?
