Can't figure out how to search efficiently for the sequence 1D-1S/2C-hearts and its variants, so I'll ask: Is there a thread?
Alternatively, how do people playing 4thSF handle all the ranges for responding hands with at 5+S-4+H? Does it make a difference in a structure where 2C is forcing?

You can bid 2♥ then 3♥ with game forcing and hearts. You can bid 3♥ over 2♣ with invitation with both majors 5/5 (some would play this has a different meaning). You can bid 2nt natural invitational skipping the 4 card heart suit since partner can bid 3♥ over that if he's 0=4=5=4 or 1=4=4=4 and couldn't reverse, and he can still show 3 spades with 3♠ too over 2nt (assuming he's like 3=1=5=4 or something).

Yes. I'm more worried about the Reverse Flannery Response hands (b/c I don't think partner will go for RFR). Responder will have to take a minor preference with many hands on which that action will be extremely distasteful, and can't pass 2C (in our structure, could be a jump shift strength hand).
U-m-m-m, I think partner will have to listen seriously to RFR....

You could play
1♦ - 1♠; 2♣
2♦ = weak with both majors or weak with spades or weak with minor support or any GF
2M = nat, invitational
2NT = nat, invitational
3m = nat, invitational
but it does mean forcing to 3♦ on poor hands. Presumably this is not a big issue for you since you already forcing to 3♣. After the 2♦ rebid, Opener bids as if partner has a weak hand with both majors. If Responder has the weak hand wanting to play in 3m after a 2M response then they bid 2NT Lebensohl-style. Higher rebids show the game-forcing hand type.
I cannot see any other (easy) way of fitting in everything you seem to require within the OP conditions. Reverse Flannery would probably be a good idea. You might also consider unloading the sequence by playing an unbalanced diamond opening, freeing up 1NT and 2NT rebids for some of these hands.
Edit: also, another plug for the ETM Gadget which can also be used in this auction.
